For #2 - that new individualized mRNA vaccine + pembro combo regime halves the risk of recurrence or death over 3 years compared to pembro alone (current standard of care), which is still an amazing improvement but not quite as dramatic as outright doubling overall survival in that population.

I'll be interested to see how that particular mRNA vaccine production process scales, and what they end up pricing it at commercially - the turnarounds they were pulling off for tumor sequencing through mRNA design to actual dose production and delivery within the clinical trial were genuinely impressive, but scaling that process and commercial pricing/affordability were always going to be the tricky points for making such highly personalized medication regimes accessible.

tl;dr I'm rooting for them to make it work, but also: capitalism often disappoints.
