I loathe and despise being referred to by solely my last name.  It gives me flashbacks to being in school.

Please use my first name (Andy or Andrew), and if there's the possibility of confusion with other Andrews then stick my surname on the end - just don't refer to me by surname.

Exactly. Referring to someone by their last name is what you do with subordinates you don't care about and children you're teaching.

I get quite a kick out of being called by my surname. Maybe that's because I fancied a few of my teachers in high school and I enjoyed getting into trouble by them.
