I'd be surprised if more depressed people than average didn't have low self-esteem. But that doesn't mean that low self-esteem causes depression, or that fixing the low self-esteem will fix the depression. It's more likely to mean (in my opinion) that depressed people have a low opinion of a lot of life, including themselves.
CBT addresses low self-esteem as part of a generally pessimistic attitude.
Have you read Good Mood by Julian Simon? I'm linking to the autobiographical section, but the whole book is online-- he broke a serious (long-term suicidal) depression by working out that the pessimism was self-imposed and changing it.
